Respected Sir,
With high esteem and regard this is to inform you that I am working as Project Coordinator in your prestigious firm since a year. My progress is satisfactory according to assessment by management along with punctuality and sincerity with my job. This is to inform you that unfortunately I am patient of Asthma. As, you are aware that season has changed and its huge problem for me to travel during changing season due to my disease as I can get severe attacks and irritation. I usually tend to stay at home until the weather conditions settle down.
I hope you will understand my health condition and I want to request you for leave of one week from 23rd June to 27th June, 2018. I will try to join back as soon as I feel my perfect health condition. I hope you will sanction me the leave and I assure you that I will cover up the official tasks side by side and will be in touch with my assistant so that it may not cause any inconvenience in official working as well. I would be highly obliged for this.
