Subject: Leave Application due to Appendix Operation
Sir,
With due respect, this is to inform you that I am suffering from severe appendix pain since a month. I have consulted a doctor and after examination it has come to front that I need to get my appendix operated as soon as possible. For this reason, I request you to sanction me leave for a week. I have taken appointment for operation it is tomorrow on 5th September, 2018 in National Hospital.
After operation, physician has prescribed me bed rest for a week. I hope to get recover and come to regular routine within this time span. Kindly consider my request and allow me the leaves. I shall be highly obliged for this.
