I am the father of Hina Ali her registration number is 34 and she is a student of 9th grade in your prestigious school. She is not well and the purpose of writing this application is to get leave for medical checkup. She is feeling really sick with pain in her right foot. I request you to please grant her two days leave and I will submit medical certificate for further necessary action, if she will need more bed rest as per doctor’s advice.
