Subject: Application for Sick Leave from Guardian
Respected Dean,
It is to notify that my daughter Huma Anwar is doing MBA in HR from this institution (6th Semester). She is feeling sick from last one week and her medical reports show that she is suffering from dangerous disease hepatitis B. She is in very critical condition and she needs proper checkup. I request you to allow her 15 days leave from university i.e (15th June till 30th June).
I hope she will recover soon by proper bed rest. I shall be highly obliged on account of her health matter.
